What will happen after I report my concern(s)?

 
 

When you submit your concern, it is logged and reviewed.

The reviewer will determine if the report meets the criteria for a compliance report.

If the report meets the criteria for a compliance report, (because it alleges a violation of law or failure to meet the mandates of a compliance agency), it will be forwarded to a designated member of the Executive Committee of The Children's Place Board of Directors.


The designated member of the Executive Committee will contact you within three (3) working days of your submission to inform you that the report has been received and is being investigated. You may be asked to provide any additional relevant information that may have been omitted from your initial Compliance report submission.

Any allegation that involves the President/CEO or involves a violation of local, state or federal laws by agency staff will be immediately forwarded to the Executive Committee of The Children's Place Board of Directors.

Investigations of suspected issues will be conducted using all available resources to gain the best understanding of the issue. These resources will include, but are not limited to:

  - Client and/or employee initial statements

  - Client and/or employee followup interviews

  - Witness interviews

  - Staff interviews

  - Any formal written records available. If said records are considered PHI (protected health information), the client and/or employee will be asked for written authorization to utilize the information solely for the purpose of investigating and resolving the matter.

Once an investigation has been deemed complete by the Executive Committee, the committee will meet and a finding will be rendered based on the available evidence. Findings are to be based on the following:


  - The validity of the initial Compliance report,
  - What action should be implemented to resolve the issue for the client and/or employee,
  - What disciplinary action, if appropriate, should be enacted, and
  - What additions or changes to current agency policy should be made as a result of this Compliance report to avoid similar issues in the future.

When completed, a report will be a made to The Children's Place Board of Directors, the appropriate agency staff, and the person who submitted the Compliance report (if a name and contact information were supplied in initial report).

If your concern does not meet the criteria for a compliance report, (because it does not allege a violation of law or failure to meet the mandates of a compliance agency), it will be forwarded to the designated agency personnel for follow up within three (3) days.
You will be informed of the steps to be taken to investigate and resolve this concern (if a name and contact information were supplied in initial report).
